Pay for two sets of metered documentary stamps (one stamped on this form and one for the Registration Certificate) at the Customer Service Center. Step 3. Present duly accomplished oath form (where your profession belongs, for verification of records and assessment at Window 14, Window 19,Window 28 or at PRC Service Center, Ever-Recto Step 4. Pay prescribed fees at the Cashier. Step 5. Sign in the Registry Book and receive your claim slip at: Window 17, Window 20, Window 27 Step 6. Claim your professional ID as scheduled. Please refer to your claim slip for further instructions. REQUIREMENTS:
New registrants should come personally to the PRC Central Office or to the nearest Regional Office to file his/her application for registration and to affix his/her signature on the registry book. In case registrant is located abroad, please find below the following requirements : 1. Take the professional's oath before the Philippine Consul. 2. Special Power of Attorney notarized by the Consul. 3. Copy of passport page showing personal data. 4. Copy of passport page showing date of entry abroad. Board Certificates shall be claimed at the Central Office or the Regional Office where the application for the same has been filed. | Steps in Applying for Nurses Licensure Examination Posted: 31 Aug 2010 08:20 AM PDT Steps in Applying for the NLE Exam Read the steps in applying for the Nurse Licensure Examination for Nurses and use the flow chart to your advantage. STEP 1 Secure Action Sheet from the PRC Customer Service Center (CSC) and accomplish the same. STEP 2 Submit the Action Sheet for processing and evaluation at any of the Processing Windows together with the following: - Original and photo/xerox copies of Transcript of Records with Special Order and Date of Graduation SCANNED PICTURES AND WITH REMARKS "FOR BOARD EXAMINATION PURPOSES ONLY". View Resolution no. 2004-200 .(Graduates of government schools and institutions/programs accredited by recognized accredited agencies under the FAAP are exempted from SO [B]). Graduates of New Schools/degree Programs must submit School Recognition and/or Permit to operate. - Original and photo/xerox copy of NSO-issued Birth Certificate (in NSO security paper; if NSO copy not very clear, bring copy from the Local Civil Registrar) - Original and photo/xerox copies of Marriage Contract in NSO security paper (for married female only;if NSO copy not very clear, bring copy from local civil registrar). - Four (4) passport size colored pictures in white background with complete name tag. - Current Community Tax Certificate (Cedula) - Other specific requirements as required by the Commission and/or Professional Regulatory Board. STEP 5 Accomplish the CAF, PERRC, and NOA. Indicate the Official Receipt number, date, and amount on the CAF. STEP 6 Proceed to any of the Issuance Windows (Window 9, 10, 11, 12) for final review of qualifications and documents and issuance of NOA, Applicant's Stub, Program of Examination, Examinee's Guide and Self-Instruction Sheet. STEP 7 Keep the Notice of Admission, Official Receipt, and the Applicant's Stub. Bring these with you on the first day of the examination. For further information, read the General Instructions at the back page of the Notice of Admission, Program of Examination, Examinee's guide and Self-Instruction Sheet. STEP 8 Bring your NOA when you verify with the PRC your school and building assignment which will be posted at the PRC premises two (2) three(3) working days before the examination date. | NLE TRANSCRIPT OF RECORDS - Resolution No. 2004-200 Posted: 31 Aug 2010 08:04 AM PDT Resolution No. 2004-200 REQUIRING APPLICANTS IN LICENSURE EXAMINATIONS TO SUBMIT TRANSCRIPT OF RECORDS WITH SCANNED PICTURES AND WITH REMARKS "FOR BOARD EXAMINATION PURPOSES ONLY" WHEREAS, the Commission issued Memorandum Circular No. 2003 – 03 dated March 28, 2003 on the "Proposed Additional Security Features on the Official Transcript of Records" to all Presidents and Registrars of Schools, Colleges and Universities and all officials and employees of the Commission. WHEREAS, the Commission has conducted consultative meetings with the presidents, deans and registrars of schools, colleges and universities on licensure admission and examination requirements including the proposal on additional security features on the transcript of records that will include the scanned picture of the graduate-applicant and the remarks "FOR BOARD EXAMINATION PURPOSES ONLY". WHEREFORE, the Commission hereby RESOLVES as it hereby RESOLVED to require applicants in licensure examinations scheduled in September 2004 onwards to submit transcript of records with scanned pictures and remarks "FOR BOARD EXAMINATION PURPOSES ONLY". FURTHER resolved that applicants who graduated several years before the issuance of this resolution shall be required to present a new transcript of records bearing the scanned picture and remarks, thus enabling the schools to keep tract of their previous graduates who will take the examinations. FURTHER resolved that schools, colleges and universities who do not have the equipment for scanning of pictures shall attach an original certification on the transcript of records stating the non-availability of equipment for scanning to be signed by the president or registrar. FINALLY RESOLVED that this Resolution upon approval shall be effective after fifteen (15) days following the publication thereof in the Official Gazette or in a newspaper of general circulation in the Philippines. Let copies of this Resolution be furnished all schools, colleges or universities, public or private, offering courses for licensure examinations and accredited professional organizations for their information and guidance.
